What You'll Do

As an LPN working with Domnovate Healthcare, you will support patient care teams within partner healthcare facilities.

Responsibilities include:

• Monitoring and documenting patient conditions
• Administering medications and treatments as ordered
• Assisting with patient assessments and care planning
• Communicating patient status with RNs, physicians, and care teams
• Supervising and supporting nursing assistants
• Participating in shift handoffs and care coordination
• Supporting patient education and discharge preparation

Qualifications

Required

• Current Licensed Practical Nurse (LPN) license
• Graduate of an accredited practical nursing program
• Current BLS / CPR certification

Preferred

• Experience in skilled nursing, rehabilitation, or long-term care
• Comfort working in team-based care environments
• Familiarity with electronic health record systems

Physical Requirements

This role may involve:

• Standing and walking for extended periods
• Lifting or assisting with up to 40 lbs
• Frequent bending, reaching, and patient assistance
• Use of PPE in clinical environments
