JOB SUMMARY
Licensed Practical Nurses (LPNs) are needed for local contract, per diem, and travel assignments at skilled nursing and rehabilitation facilities across Utah. Current openings are available in Salt Lake City, West Valley City, Provo, Ogden, Layton, St. George, and Logan, with additional opportunities throughout the state.
This role offers flexible scheduling, competitive pay, quick start opportunities, and assignments in skilled nursing, long term care, post-acute, and rehabilitation settings.
PAY RANGE
Local contract and per diem assignments: $30.00-$38.00 per hour depending on shift, experience, facility, and location
Travel assignments: $1,100-$1,500 per week equivalent blended rate
Higher rates may be available for nights, weekends, and select assignments
RESPONSIBILITIES
- Provide direct patient care under the supervision of a Registered Nurse or physician
- Administer medications and treatments as ordered
- Monitor patient condition and report changes in status
- Document care, observations, and treatments accurately in the medical record
- Assist with patient assessments and care planning
- Communicate effectively with nurses, physicians, therapists, and care team members
- Follow facility policies, state regulations, and clinical standards of practice
- Support quality care in skilled nursing, rehabilitation, and long term care environments
REQUIREMENTS
- Active Utah Licensed Practical Nurse (LPN) license or ability to obtain Utah licensure
- Current CPR or BLS certification
- At least 1 year of LPN experience preferred
- Experience in skilled nursing, long term care, rehabilitation, or post-acute care preferred
- Strong clinical, documentation, and communication skills
- Ability to work assigned schedule and meet attendance expectations
PREFERRED QUALIFICATIONS
- Skilled Nursing Facility (SNF) experience
- Long Term Care (LTC) experience
- Rehabilitation or post-acute care experience
- Experience with electronic medical records including PointClickCare or similar systems
